Greg Puklicz, who has served as President since December 2021, has been appointed Chief Executive Officer (CEO). Long-time CEO Richard K. Blaylock will continue to provide leadership, governance and insight in his role as Chairman of the Board, maintaining the family legacy started by Charles C. Blaylock in 1984.
In his new role as CEO and President, Puklicz will oversee all corporate operations, strategic initiatives, and the continued expansion of the 12 Oaks regional footprint.

Puklicz joined 12 Oaks Senior Living in 2018 as Chief Financial Officer before being named President in late 2021. During his tenure as President, Puklicz successfully expanded the company’s portfolio to over 40 communities. Crucially, he led the build-out of new corporate verticals to provide a comprehensive, full-service capability that spans clinical care, sophisticated sales and marketing support, and specialized life enrichment programs. Backed by a best-in-class back-of-house operational capability, 12 Oaks has solidified its reputation for driving financial efficiency and clinical excellence for its owners and investment partners.
